The short answer: count the fixtures. If one fixture is misbehaving and everything else in the building drains normally, the problem is in that fixture’s own pipework. If several are affected at once, the problem is in the drain they share. Almost everything else follows from that one distinction.
It sounds too simple to be diagnostic. It is not — it is the single most useful thing you can establish before anyone attends, and it is the difference between a contained job and a main-line job.
The test
Do not flush or run anything into the affected fixture. Instead, use a different one. Run a basin tap for thirty seconds, then run a shower. Then go back and look.
If nothing changed at the affected fixture, the rest of the system is accepting water normally. If the water level rose, bubbled, or gurgled, water you put somewhere else reached it — which means both fixtures are meeting the same restriction downstream.
That is the whole test. Everything below is a longer version of it.
Case 1: one fixture only
One sink, basin, shower or toilet is slow or blocked. Everything else works.
What it means. The restriction is in that fixture’s trap or the branch pipe serving it, before it reaches the drain everything else uses. It is the smallest version of this problem.
Common causes. Hair and soap in a shower or basin; fat and food waste in a kitchen sink; something that should not have been flushed in a toilet.
What follows. A contained clearing job. Worth noting if it is the second or third time for the same fixture — that changes it into case 6.
Case 2: several fixtures at once
More than one fixture is slow, and they are on different branches — a kitchen sink and a bathroom shower, say.
What it means. Fixtures with separate branch pipes failing simultaneously points at the thing they have in common: the shared drain. The blockage is downstream of where their branches join.
Common causes. Accumulated build-up in the main line, roots, a damaged or displaced section, or a substantial obstruction.
What follows. A blocked sewer drain is the situation to treat this as. Clearing one fixture will not help — that fixture was never the problem.
Case 3: gurgling across fixtures
Running one fixture makes a different one gurgle. A toilet bubbles when the washing machine drains. A floor waste burps when the shower runs.
What it means. Air being displaced past a restriction. Water moving down the line pushes air ahead of it, and where the line is partly blocked, that air escapes through the nearest opening — which is a different fixture.
Why it matters. Gurgling frequently shows up before anything actually backs up. It is an early warning, and acting on it is considerably cheaper than acting on the overflow that follows it.
What follows. Treat it as a main-line problem in its early stage.
Case 4: something is overflowing outside
Waste water is discharging from an overflow relief gully in the yard, or surfacing around a drain.
What it means. The main line is blocked and the system is releasing where it was designed to release. The gully sits lower than the fixtures indoors specifically so that this happens outside rather than in your bathroom.
What to do immediately. Stop using water everywhere on the property. Every flush and every load of washing adds volume to a system that has nowhere to put it, and it is the most reliable way to turn an outdoor overflow into an indoor one. Keep people and pets away from what has surfaced — it is a health risk, not just a mess.
What follows. This is the urgent version of case 2.
Case 5: only when it rains
Water pools in the yard, a pit surcharges, or a downpipe overflows — but only during or just after rain, and no waste water is involved.
What it means. This is the stormwater system, which is an entirely separate set of pipes from the drain carrying waste water. It has its own inlets, its own pits, and its own blockages.
Why the distinction matters. Work done on the sewer drain does nothing here. They are different pipes. A visit spent on the wrong system is a visit spent, and the problem is still there next time it rains.
What follows. Blocked stormwater drains covers the causes and what to check first. Surface inlets choked with leaves are worth ruling out before assuming the problem is underground.
Case 6: it has happened before
The drain was cleared, worked normally, and has now blocked again — often in the same place.
What it means. The first blockage was not the whole story. Something in that section is catching material: roots through a cracked joint, a displaced section, a sag holding water and solids, or deteriorated pipe.
What to look at. The interval, more than the count. Three blockages spread over a decade is not obviously a pattern. Three with the gaps shortening is a trend, and the trend says the restriction is getting worse.
What follows. This is the situation a CCTV drain inspection exists for. Clearing it again treats the symptom of something that has not changed; a camera establishes what is actually there. Either it finds a defect that can be repaired, or it confirms the pipe is sound and the cause is what is going into it — and both of those end the guessing.

What to have ready when you call
Which fixtures are affected and which are not. Whether rain is involved. When it started. Whether it has happened before, and roughly how long ago. Whether anything has surfaced outside. If you know where the drain inspection opening is, mention that too.
Those six things take under a minute to gather and they change the conversation from “the drain is blocked” into something specific enough to act on.
